What is needed from you: To submit new patient's forms and a short nutrition questionnaire (emailed after booking). To implement improvements and changes as per therapists' recommendations. Motivation, desire to achieve your goals and belief in your success! Don't worry, this will not take over your life as the goals will be set specific to your needs and abilities. NOTE: AS PER OUR POLICY, IN CASE OF A LAST-MINUTE CANCELLATION (LESS THAN 24 HRS IN ADVANCE), YOUR $75 DEPOSIT WILL BE KEPT AS A CANCELLATION FEE. IF RESCHEDULING YOUR APPOINTMENT FOR A DIFFERENT TIME ON THE SAME DAY, THE CANCELLATION FEE WILL BE WAIVED, BUT IS SUBJECT TO OUR AVAILABILITY.
